Please read the following instructions before you proceed;

1 - This form is only for those candidates who have neither appeared for 11th or 12th examination nor
      enrolled in BISE Lahore.
2 - If any candidate has appeared for 11th / 12th examinations earlier or enrolled for Higher Secondary
      School Certificate (Intermediate), he need not to re-enroll.
3 - Already appeared / registered candidate can use his already allotted enrolment number for 
      admission process at the time of admission.

Bay-Form Number and Date Of Birth Cannot be changed after you press the proceed button, So please enter these carefully as per NADRA record.
